I'm Dr. Jac Constantino, a Doctor of Nursing Practice and board-certified Psychiatric Mental Health Nurse Practitioner with additional board certification in Adult-Gerontology Primary Care. My dual certification allows me to understand the close connection between mental and physical health, providing a comprehensive approach to psychiatric care that considers the whole person — not just a diagnosis.

With years of experience treating adults across a wide range of mental health conditions, I specialize in comprehensive psychiatric evaluations, thoughtful medication management, and individualized treatment planning. I work collaboratively with each patient to develop evidence-based treatment plans that align with their goals, values, and lifestyle.

I believe the best psychiatric care happens when patients feel heard, respected, and actively involved in their treatment decisions. Appointments are intentionally designed to provide the time needed to fully understand your concerns, answer your questions, and develop a treatment plan that is personalized — not rushed.

As a private practice, we are able to offer a level of personalized attention that larger clinical settings often cannot. Appointments are unhurried, communication is direct, and your care is never handed off to someone who doesn't know your history.

We currently offer both in-person and telehealth appointments, making it easier to maintain consistent care regardless of your schedule or location.
